WHAT IS SLEEP?

We spend a third of our life sleeping. But as it is not perceived with conscious mind, it remains an uncharted territory. So sleep is mysterious. And so is ignored.

The wild imagination of creative minds made it even more mystic. But not anymore. Scientists have deciphered a lot about the biologic basis of sleep. As the scientific march advanced, we eventually entered into an era of sleep medicine, where we are able diagnose and treat myriads of sleep disorders today. Consumer sleep aids too continued to flourish, all to help you sleep better.

We now know that, many of the disease processes that overtly manifest during wakefulness, actually starts much earlier during sleep but remain undiagnosed.

Frequently Asked Questions

Am I having a sleep disorder?
If you have a sleep disorder, you will have the following symptoms or signs: you can't have a good night's sleep (either you can't fall asleep easily, or your sleep breaks easily and repeatedly, or you wake up too early), or you have excessive sleepiness during day, don't feel refreshed in morning, fall asleep here and there, or you may have disturbing dreams or perhaps you may not have any trouble but your bed partner do have like loud snoring…the events you may not remember at all.
When should I consult a sleep specialist?
If you have any of the above discussed symptoms of your own or reported by your bed partner, you should seek professional help. Further, if you have uncontrolled high blood pressure, high blood sugar, unexplained fatigue or tiredness, unexplained heart disease or stroke, or if you have memory or anger issues, if you unexpectedly gain weight or are already obese, you should consider to consult a sleep specialist.
I have a habit of take sleeping pills. Can I come out of it?
Recommended and scientifically validated first line treatment of chronic insomnia is not sleeping pills, but to change your perception about sleep by challenging your thought process through cognitive behavioural therapy. Long term use of so-called sleeping pills causes tolerance and dependence. Most of such pills are not true sleeping pills but have sleep as their side-effects. So, if you have trouble sleeping and want to come out of your habit of sleeping pills, consult a sleep specialist or clinical psychologist experienced in CBT for insomnia.
Where should I go for a sleep study?
Post-pandemic, there is a paradigm shift in sleep testing strategy favouring home sleep study over in-lab study using a portable polysomnograph or a PAT based portable device. However, it should be kept in mind that, the gold standard of sleep testing is still in-lab supervised polysomnography. However, the abbreviated home sleep study or portable sleep study is more convenient for the patient and for professionals too. But, such portable sleep study is not to be used indiscriminately. It is reserved only for carefully selected patients having high probability of pure sleep apnea, without any other comorbid sleep or cardio-respiratory disorder. Any patient having complicated issues should undergo in-lab polysomnography.
Are mobile sleep trackers apps reliable?
Sleep trackers available as wearables like smartwatch or as simple as smartphone apps. They usually rely on your movement to measure sleep. Although not clinically validated and as accurate as a clinical polysomnography, they can be relied upon as far as gross information on sleep quantity and quality is concerned. At least these are not inferior to self-reported sleep logs or subjective questionnaires. You can have a good insight about your sleep using these trackers. But, you should use that insight for improvement of your sleep, and should not to be obsessed with worries about poor sleep.
I was prescribed CPAP for my sleep apnea. But I am very apprehensive to use it. May I get any help?
CPAP is the most commonly prescribed treatment for sleep apnea. If you have issues with your device or the mask, get in touch with your doctor or supplier. If you are too apprehensive, you may need support and re-assurance from sleep physician or a sleep technician or a sleep health coach. However, you may also consider visiting a sleep physician to be assured that your prescribed CPAP pressure or mask are optimum. There is a possibility that, you may also be treated with alternate treatment modalities like position therapy, jaw repositioning or some surgery, if clinically suitable.
